For the homepage title in and other page titles in the browser tab title (not sure of what to call it exactly), I’m getting repeats. You can check it out at: http://www.mashupasia.com

From there, hover over the tab to see the title of the page and you’ll see the duplication. It occurs on all the pages. How can I get rid of this? I tried to tweak the header.php, but can’t find out what to comment out.

Nevermind! I got it fixed finally. The commenting out was being a little difficult for me.

    Where were the repeats showing?

The repeats were in the Page Title. So in the top of the browser windows themselves. I basically commented out all the code in and put in:

    <title><?php wp_title(''); ?></title>

So it now no longer does duplicates.

The theme should display : “post title | site name” on posts/pages and “site name – description” on the homepage.

Maybe you have some kind of SEO plugin that might be modifying this?

Yes, I have the WordPress SEO plugin, but all the “Title” fields in the plugin settings are blank. That’s probably the issue.

A quick check with plugin disabled would be a good test.

Yep, it was WordPress SEO that led to the double title issue.
